RN TEAM LEADER / SUPERVISOR
The team leader is responsible for implementation, coordination, and supervision of all aspects of the delivery of patient/family services, consistent with agency policies.
--Responsible for the supervision of direct patient care services and the interdisciplinary team component of the agency program and for the planning, coordination, and supervision of delivery of care to patients/families.
Achieves objectives of the hospice by directing the services provided by or arranged for by the hospice by:
• Demonstrating knowledge of and adherence to hospice policies.
• Developing and implementing procedures and a system of staff communication that ensures coordinated implementation of treatment plans and utilization of necessary services to achieve comprehensive patient care.
• Planning and directs supervision of hospice personnel.
• Demonstrating knowledge of and adherence to state regulations for hospice agency operations.
Responsible for the development of personnel qualifications by:
• Participating and/or initiating review and revision of personnel policies.
• Establishing criteria and procedures for the recruitment, selection, promotion, evaluation and termination of employment of hospice personnel;
• Providing for orientation of all new employees.
• Ensuring an inservice training program consistent with staff and patient needs.
Responsible for appropriate personnel assignment by:
• Directing the assignment of qualified care personnel based on patient requirements.
--Responsible for a staffing plan to meet the objectives of the hospice by:
• Maintaining an adequate number of qualified personnel.
• Maintaining written service agreements for appropriate services, when required.
--Ensures the availability of adequate supervision of service during service hours by:
• Delegating responsibility to appropriate registered nursing personnel in the absence of the hospice coordinator.
--Participates in liaison activities in the community by:
• Assisting in providing the public with appropriate and adequate information regarding the hospice and its services.
--Under the direction of the Director of Patient Care Services, develops, directs and monitors the Quality Assessment and Improvement and Utilization Review Programs by:
• Ensuring the program meets or exceeds state regulations.
• Implementing the utilization review process according to state regulations.
--Maintains a positive and professional image by:
• Demonstrating self-directed learning to meet professional development and licensure requirements.
• Participating in professional associations to assure current knowledge of regulations and guidelines.
--Complies with all policies and procedures related to the Corporate Compliance Program.
Qualifications
--A current California Registered Nursing license, preferably with a B.S. degree in Nursing,
--At least two years of relevant experience in hospice, home health, or community health experience within the past five years.
--Demonstrated ability to manage all aspects of terminal care with specific expertise in crisis intervention, family counseling, and staff supervision.
--A Tuberculin Skin Test or chest x-ray within six months prior to employment or within 14 days of the date of employment and every year thereafter.
--A health examination within six months prior to employment or within 14 days of the date of employment.
--A reliable automobile and current California Driver\'s License.
