RN, Navigator, Cancer, Cancer Center — Glendale
Job Description
Acts as a patient advocate. Guides patients through the oncology care system, establishing contact with newly diagnosed cancer patients and/or family member, or caregiver. Understands the clinical care options for patients and directs them to healthcare services within the organization, at outside facilities and within the community for timely treatment and survivorship.
Actively identifies and addresses population and patient specific barriers to care that might keep the patient from receiving timely and appropriate treatment. Connects patients with resources, healthcare and support services in their communities.
Supports the physician and interdisciplinary team in facilitating patient care with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction. Implements day-to-day care management interventions for identified clinic patients.
Qualifications
Education and Work Experience:
- Bachelor's Degree in Nursing (BSN): Preferred
- Oncology nursing experience: Preferred
- Experience in healthcare screening or cancer control activities: Preferred
Licenses/Certifications:
- Registered Nurse (RN) licensure in the state of practice: Required
- Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification: Preferred
- Oncology Certified Nurse (OCN): Preferred
