Nurse Coordinator describes a variety of specialized nursing positions that provide professional support, planning, coordinating, and facilitating patient care for selected patient populations across the continuum of care, ensuring compliance with provider orders and established policies and procedures. This position utilizes the nursing process without providing direct patient care. Nurse Coordinators provide coordination and care for patients requiring assistance and support in accessing and managing their health care needs. As an integral member of the care team, they collaborate with other health care professionals to ensure continuity of care and coordination of a wide variety of patient care services. 

Qualifications

• Bachelor’s degree or equivalent from an accredited college/university Required
• Two (2) years of progressively responsible and directly related work experience
• California Registered Nurse (RN) license
• Basic Life Support (BLS) certificate for providers from the American Heart Association
