Job Description

Primary responsibility is flexible hybrid with a variable schedule on-call coverage for coordination of/liaison between transplant teams (Kidney/Pancreas, Liver/Small Bowel, Heart, and Lung), patients, hospital departments, and organ procurement organizations for transplants.  Secondary responsibilities encompass all aspects of clinical coordination of the transplant recipient involving all phases of the transplant process, from initial pre-transplant referral (including urgent transfers) throughout the post-transplant patient follow-up phase.

This responsibility involves the coordination of the interdisciplinary medical care delivery system under the direction of the transplant program physicians, nurse manager, and nursing supervisors to facilitate liaison functions between the patient/family and the medical staff, referring physicians, and insurance companies.

Additionally responsibilities include: triage of patient and family calls, coordination and workup of incoming new patient referrals that occur outside business hours, a working knowledge of UNOS data entry, and individualized patient/family teaching and assessment of appropriate medical intervention for effective use of resources and quality assurance. Transplant donor/recipient coordination and liaison function with organ procurement agencies.

Qualifications

1.     TC I:  Current California Registered Nurse license, graduation from an accredited nursing program and a  BSN or Master’s degree with  5 years of relevant RN experience with a combination of critical care and  transplant coordinator experience (required).

2.     TC II: Current California Registered Nurse license, graduation from an accredited nursing program and a BSN or Master’s degree with a CCTC required and 5 years of relevant RN experience with a combination of critical care and transplant experience (required).
