Sr. Director, Care Management, RN
Job Description
The Senior Director of Care Management is responsible for the successful development, execution, and oversight of care coordination initiatives through the Care Management departments at all PIH Health Hospitals.
Responsible for ensuring quality and integrated clinical patient care and physician/nurse relationships, support organizational financial viability, operational effectiveness, patient satisfaction and strategic initiatives. Ensures safe transitions in care through the development and enhancement of strong partnerships with the post-acute care network.
Oversees the metrics reflective of effective care management functions and processes.
Qualifications
Required:
- Active California RN license or Social Work LCSW license
- Minimum of five (5) years’ experience in acute care hospital, case management, social work, utilization review, or discharge planning in an acute care hospital
- Minimum of two (2) years leadership and department management experience.
Preferred:
- Master's degree in nursing, health care administration, business, or related field is also required
- Case Management Certification
